Present a data-capture form configured via the Forms service.

How do I create this Element?

From inside an Event, press the [ + Element ] button to open the Element selection menu and select the Element you want to create. Elements are placed on the timeline of an Event, with Timing properties based on the Element type. Read more about Timing here.

Element setup

When creating this Element you will see the following tabs in Studio to configure your Element:

Content

Title - Main headline typically serving as the subject to help audience quickly grasp its context

Subtitle - A subordinate title usually accompanying the main title to provide more context/instructions

Image - Displayed boldly at the top of the title to capture the audience attention and to provide even more context

Video (embed code) - Serves same purpose as Image but can takes precedence when there's a pre-existing image

Aspect Ratio - Choose an aspect ratio depending on desired visual impact and platform requirements

Form - Select the preferred form from the dropdown of all forms created in Project
